Proof of Residency

Attached is the proof of residency form that Rocky Boy Health Center is asking eligible enrolled members to fill out.  This form is a requirement for PRC services which includes referrals.

Patient Visits

When Visiting Rocky Boy Health Center:

​

  • Arrive 15 minutes early so that you may complete Patient paperwork

  • Know any health conditions you have currently or have had in the past

  • Know any major health conditions that your family (parents, siblings, grandparents) currently has or has had in the past

  • Bring your health insurance card if you have one (you will need this card at every visit).  If you do not have insurance, we will help you apply for it.

  • Bring a list of any medications you are taking (or bring the medications themselves)

  • Prepare any questions you want to ask about your health or our services

  • The name and phone number of your pharmacy, if you currently use an alternate to the RBHC Pharmacy

 

During your visit, we will:

​

  • Help you fill out necessary forms or obtain necessary health records

  • Explain how to make appointments and get access to all of our services

  • Make any appointments you need with other departments or services at RBHC

​

By the end of your visit, you should know:

​

  • The answers to most of your questions; if not, please ask! You can also schedule a follow-up visit, or check with registration to determine who can help answer many questions

  • What your Treatment Plan is (medications, referrals, other services, follow-up visits)

  • What medications you will be taking, how to take them, possible side-effects, and why you are taking them

  • The date and time of any lab tests or follow-up appointment

To make an appointment:

Due to COVID-19 protocol, Walk-In visits are not allowed.

 

Call our Main Line: 406.395.4486 to make an appointment. Appointments are required to be seen at Rocky Boy Health Center unless your condition is an emergency. Remember to check in at Registration for your appointment at least 15 minutes before the scheduled appointment time.

If you need to cancel an appointment:

Please call us at least 24 hours in advance if you cannot make your appointment so that we may offer that appointment to another patient and ensure you are rescheduled to serve your care needs.

Please keep your records updated; tell us about changes in your address, telephone number, email, or Insurance information.

Welcome to Rocky Boy Health Center. We are grateful you have chosen us for your care and honored to be your community health center. The following information will help you prepare for an appointment to streamline your experience and enable us to best serve you.

Patient Rights and Responsibilities

Rocky Boy Health Center recognizes that health care is your right and is responsible for delivery of that care with high degree of professionalism.


You Have the Right to:

  1. Considerate, respectful care always and under all circumstances

  2. Know the identity and professional status of your care team

  3. Privacy concerning your care plans, treatment, and records

  4. Expect safe and clean conditions at any Rocky Boy Health Center facility

  5. Receive complete information of services and treatment in terms you can understand to make informed decisions and participate in your health care

  6. Refuse treatment after receiving full information necessary to make that decision

  7. To have someone assist you and provide support while seeking services unless your care team determines that the person may interfere with your rights and safety or those of others

  8. To make, review, or revise any Advance Directives and to appoint someone to make health care decisions for you if you are unable to do so

  9. Refuse the presence or services of a health care student or anyone not directly involved in you care

  10. To receive proper pain assessment and management

  11. Receive full explanation of Purchased Referred Care Services, Medicare/Medicaid, and/or Third-Party insurances and how it may affect the scheduling of any elective surgeries or treatment

  12. Be informed of Rocky Boy Health Center rules and regulations that apply to patient conduct, processes, including those for resolution of patient complaints

  13. Give Feedback or concern with Rocky Boy Health Center without it affecting the quality of your care

  14. Request a second opinion from another practitioner at Rocky Boy Health Center

  15. Seek additional health care services and/or specialty consultation outside the Rocky Boy Health Center without a referral at your own expense
     

You Have the Responsibility to:


1.    Provide the most accurate and complete information of past history, medications, hospitalizations, and other matters relating to your health.  

2.    Provide up to date contact information 

3.    Actively participate in your healthcare

4.    Share feedback and suggestions to assist us in providing the best care possible.

5.    Cooperate and actively participate with the care plan made by you and your physician; including any instructions of nurses and allied health personnel as they assist with the plan of care while following applicable rules and regulations.

6.    Accept responsibility for your actions if you refuse to follow your care plan.

7.    Accept that laws, safety issues, and ethics may affect our efforts around pain management

8.    Follow Rocky Boy Health Center rules and regulations regarding patient care and conduct

9.    Be considerate, respectful, and kind to all of Rocky Boy Health Center’s staff and patients.

10.    Ensure your support person follows all Rocky Boy Health Center rules and regulations and is considerate and respectful of Rocky Boy Health Center personnel

11.    Request further explanation if  you do not understand your care plan, billing services, or privacy practices

12.    Be aware of the type of care provided and follow all policies and procedures for the area you are visiting:

  • Outpatient Clinic: Provides complete medical services including in-depth lab work, x-rays, consultations, referrals, and ancillary services (Optometry, Dental, Physical Therapy, etc.)

  • Urgent Care Rooms: Conditions that medical staff have triaged as needing evaluation/management within 24 hours.

  • The COVID/Respiratory Clinic: Provide evaluation and testing for those patients who have symptoms of COVID-19 in a separate area to reduce exposure to other patients and staff
